Administering Oxygen in COPD

What is the appropriate amount of oxygen to deliver to a patient with COPD? You must consider the hypercapnic and hypoxic drive and the tolerance to CO2. Giving oxygen to a COPD patient over a long period of time can decrease the hypoxic drive. If you increase the hypoxic drive, which may already be high in a COPD patient, it may cause them to raise their CO2 levels even higher. You don’t generally want to use a lot of oxygen in a COPD patient. During an exacerbation maintain a p02 of 60-65 mmHg and a saturation of 90-92%, monitor closely and wean slowly when on oxygen at home. If the respirations start to go down, decrease the oxygen, but during exacerbations, give them what they need. Treat acute hypoxia. If being treated in the hospital, goals may be a little higher.
